Dr. Marks is currently accepting new patients for virtual therapy. Recurring appointments are available on a weekly or biweekly basis. Recurring, monthly, appointments are not available. These slots are filled one at a time, per availability at the time of scheduling.
Once you’ve scheduled your appointment, please be sure to follow the directions under the heading "For You to Do Before Your Initial Appointment."
What to Expect:
The initial appointment is an hour long diagnostic visit. Dr. Marks will gather information regarding your current and past symptoms as well as your biological and social history in order to formulate a diagnosis and tentative plan for treatment. After the visit Dr. Marks will provide you with a brief overview of the discussion and any diagnostic impressions.
Dr. Marks is an in network provider with Blue Cross/Blue Shield and United Health Care, and will bill insurance for these two agencies. Please note: Dr. Marks does not accept the United Health Care Community Plan (Medicaid) at this time. If you have a copay, please be prepared to pay at the time of your appointment. If you do not have mental health coverage with either of aforementioned insurance agencies, the cost for a diagnostic visit is $210. Payment is expected at the time of your visit. Dr. Marks will then bill your insurance company as an out of network provider and they will reimburse you at your plan's out of network rate. You may call your insurance company to find out how much your insurance plan pays out of network providers.
Due to the COVID-19 pandemic, most therapy and diagnostic appointments will take place via telehealth (zoom). Please review the notice on the website under the COVID-19 tab for details about the use of telehealth communications within this practice. You will receive an email from "therapy appointment" asking you to create login credentials. This is the platform that we will use for your therapy visits. You must create your credentials within 24 hours of receiving the email. If you are unable to do so, please contact me via email when you are ready to create your credentials and Dr. Marks will resend the email so that you may complete the process.
For You to Do Before Your Initial Appointment:
Please retrieve the appropriate intake packet for your needs (child or adult) or wait for a digital copy to be sent to your email via "adobe sign". It is located under the “new patient” tab. There is a release of information available for download as well, please complete it with information for any agencies, doctors, or individuals with whom you would like for me to release your protected healthcare (mental health) information. You may make as many copies of the release form as you would like. Consent to release information may be revoked at any time. You may return the registration documents via email or fax. You may also print and bring them in for the visit. Please be sure to email or fax a copy of your insurance card. If you have any previous documentation (psychological reports of any kind) please bring them or provide them securely via fax or through the therapyappointment.com portal. Lastly, if you are currently taking medications, please bring a list of your medications and be sure to fill out a release of information for any doctor who prescribes psychotropic medications for you.
