Once you’ve scheduled your return to work evaluation, you will receive an email with screening tools and questionnaires you should complete before your initial appointment.
You must also provide a copy of your full job, description and responsibilities (preferably from your HR department).
Once the preliminary documents have been received, Dr. Marks will conduct a one-hour interview to ask questions about your functioning before, during, and after your workplace event.
When the evaluation is complete, Dr. Marks will write a letter recommending you return to work in one of the following capacities:
Full duty with recommended accommodations
Modified or Restricted duty
Unable to return to work/Further treatment or evaluation necessary
The cost of a return-to-work evaluation is $225.
For individuals wishing to have a complete diagnostic report, an additional charge of $195 is added to the cost of the evaluation.
Requests for complete reports must be made at the time of scheduling.
